We value our relationship with you and want to ensure its success through mutual understanding of our patient policies. Acceptance of a surgical or procedural quote, or scheduling an appointment means that you acknowledge, understand and accept the terms and conditions below.
Consultations& Office Visits
A $50 fee is required for consultations. This fee will be applied as a credit toward the cost of your surgery/procedure when you are ready to schedule.
Unfortunately, no shows and late cancellations are a serious issue that can be costly to the practice and prevent other patients from being able to be seen. Therefore, we require a credit card to reserve your consultation. You will not be charged until the day of your appointment, however, your card is kept on file and is subject to the full ($50) fee if you fail to provide advanced (24hr) notice for cancellation.
Once you are established with us as a patient, we do not require a credit card to schedule office visits. However, a credit card may be required if you miss three or more appointments without providing advanced (24hr) notice and a $25 cancellation fee will be charged for any further late cancellations and no-shows.
If you are more than 15 minutes late for your appointment, you may be asked to reschedule.
All cosmetic service fees (i.e. injectables, facials, etc) are due upon treatment.
A 10% non-refundable booking fee is due at the time of scheduling and will be applied toward the overall cost of the procedure. Any remaining balance is due 14 days prior to the date of your procedure. If your procedure is scheduled less than 14 days prior to the procedure date, all fees are due at the time of scheduling and all normal cancellation fees apply.
Should you need to reschedule your appointment, you must give 7 days’ notice. If your procedure is rescheduled with less than 7 days’ notice, a $250 rescheduling fee will apply.
Office procedure cancellations create serious scheduling difficulties. The office surgical suite, surgeon and other staff are reserved and scheduled in advance. Therefore, please understand the importance of respecting our cancellation policy.
Office Procedure Cancellation Policy
Booking fees are NON-REFUNDABLE. Fifty percent (50%) of the TOTAL cost of the procedure is NON-REFUNDABLE if you cancel less than 7 days prior to your procedure. 100% of the TOTAL cost of the procedure(s) becomes non-refundable if your procedure is cancelled 48 hours (or less) prior to your scheduled procedure date.
A 10% booking fee is due at the time of scheduling and will be applied toward the surgeon’s fee. The remaining balance for the surgeon’s fee is due 6 weeks prior to the date of surgery. If your procedure is scheduled less than 6 weeks prior to the date of surgery, all fees are due at the time of scheduling and all normal cancellation fees apply.
Anesthesia fees are due at your pre-op visit, approximately one week prior to the date of surgery. Surgical facility fees are may be paid at any time, including the day of surgery. Facility & anesthesia fees are charged based on a fee schedule provided to us by their offices and are subject to change at any time. Please note that facility and anesthesia fees are paid directly to their billing departments, not to our office.
Should you need to reschedule a surgical procedure less than 6 weeks’ prior to your surgery date, a $250 rescheduling fee will apply. Surgical procedures cannot be rescheduled less than 3 weeks’ prior to your surgery date and will be subject to normal cancellation policies instead.

Surgical Cancellation Policy
Booking fees are NON-REFUNDABLE. Fifty percent (50%) of the surgeon’s fee is NON-REFUNDABLE if you cancel your surgery (or any portion of your surgery) 8 – 14 days prior to your surgery date. One hundred percent (100%) of the surgeon’s fee is NON-REFUNDABLE if you cancel your surgery (or any portion of your surgery) 7 days or less prior to your surgery date.
Payments, Return & Refunds
We accept cash, personal check, cashier’s check, all major credit cards, Care Credit and Patient Fi.
All sales are final. In the case of documented allergic reaction or clearly defective product, exchanges can be made within 14 days for an alternate product or skincare credit.
