Envision Physician Services is a national group of care providers. They work with the North Florida Surgical Pavilion to provide the Anesthesia services for your upcoming surgery.
The quoted price you were provided for their fee is based on a PRE-payment basis. It is extremely important that you pay this fee directly to their billing department in advance of your surgical date.
IF ANESTHESIA SERVICES ARE NOT PAID PRIOR TO YOUR SURGICAL DATE THEIR FEE WILL GO UP SIGNIFICANTLY!
Please follow the instructions below to PRE-PAY for your anesthesia services:
- Call Envision 1-800-296-2611; select option 3
- Please provide them with the following information
- Surgical Center: North Florida Surgical Pavilion
- Your Surgical Date
- Amount you are paying (you can find this on your quote or call our office)
- Your personal information (name, DOB, email, etc)
You will be provided with an emailed receipt for your payment. We recommend you retain this receipt as a record of your pre-payment.
Sadove Plastic Surgery does not control fees for Anesthesia services and we have no recourse to adjust the fee you are charged before or after surgery.
